"Rise Again" Monster Bonus Action card

Does the "Rise Again" Monster Bonus Action card have the same graphic design as the other Monster Bonus Action cards, or does the Necronomicon object card double as the Rise Again! card (when not in use in the 5th scenario which uses it)??
